Portal Registration Instructions

1. Click on the "Sign Up" button.

2. Enter your Name, Email Address, solve the Captcha, and click Submit.

3. You will be prompted to Sign in. Click Send code to send a one-time code to your email.

4. Enter the code that was sent to your email.  Click Sign In.

5. On the permissions requested by screen, click Accept.

6. Enter your phone number to receive an SMS Text for Authentication.  Click Next.

7. Enter Code from SMS Text Message.  Click Verify.

8. Verification is now complete, and you will be redirected to the CDPH Portal.
